目的

探讨三维重建技术对提高腹膜后肿瘤临床教学质量的作用。

方法

选取上海长海医院外科基地的规范化培训住院医师共60名,随机分为观察组与对照组,每组30名。对照组单纯使用CT、MRI等影像学资料进行教学,观察组应用三维重建技术辅助腹膜后肿瘤教学,对比两组取得的考试成绩,采用问卷调查形式评价教学效果。

结果

对于腹膜后肿瘤毗邻解剖关系相对简单病例,与对照组相比,观察组的总成绩及各考试评分项目有优势,但差异未达到统计学意义;对于相对复杂病例,观察组在以下几项成绩具有显著优势,差异有统计学意义,包括考试总成绩(P=0.001),评估侵犯血管数量(P=0.002),评估侵犯血管名称(P=0.001)。在以下几项成绩有优势,但差异未达到统计学意义,包括评估肿瘤大小(P=0.640),评估需要联合切除的脏器(P=0.069),评估术中出血量(P=0.118)。调查问卷显示观察组对教学的评价显著高于对照组(P<0.05)。

结论

使用三维重建技术辅助腹膜后肿瘤临床教学,能够帮助规培医师迅速掌握腹膜后解剖基础,提高影像学读图能力,提高临床教学效率与质量,尤其在腹膜后肿瘤与毗邻解剖关系较为复杂的病例中显的尤为突出,值得在教学活动中推广。

Objective

To explore the effect of 3D reconstruction technology on improving the clinical teaching quality of retroperitoneal tumors.

Methods

Sixty surgical residents received standardized training from Shanghai Changhai Hospital were selected and divided into the control group (n=30) and the observation group (n=30) using a random number table method. The observation group applied 3D reconstruction technique to assist in teaching retroperitoneal tumors, while the control group used CT, MRI and other imaging data alone for teaching. Compared the test scores achieved by the two groups and the teaching effect was evaluated by the questionnaire survey.

Results

For the cases with relatively simple anatomical relationships of retroperitoneal tumors, the observation group had an advantage in the total score and each examination score item compared with the control group, but there was no significant difference. For the relatively complex cases, the observation group had significant advantages in the following achievements, with significant differences, including total examination scores (P=0.001), assessment of the number of blood vessel invaded (P=0.002), assessment of the name of blood vessels invaded (P=0.001). There were advantages in the following scores of observation group, but no significant difference, including assessment of tumor size (P=0.640), assessment of organs requiring combined resection (P=0.069), assessment of intraoperative hemorrhage (P=0.118). The questionnaire showed that the evaluation of teaching in the observation group was significantly higher than that in the control group (P<0.05).

Conclusions

Three-dimensional reconstruction techniques assist in clinical teaching of retroperitoneal tumors, which can help standardized training residents quickly grasp the fundamentals of retroperitoneal anatomy, improve imaging reading ability, and increase the efficiency and quality of clinical teaching, especially in the cases with relatively complex anatomical relationships of retroperitoneal tumors, and deserve to be promoted in teaching activities.
