时至今日,手术仍是治疗前列腺增生最有效的方式,尽管如此,任何手术方式都可能会造成不同程度、不同比例的术后尿失禁。术后创伤形成的瘢痕破坏了尿道黏膜的闭合机制,术中对括约肌的牵拉压迫等都是术后发生压力性尿失禁(SUI)的原因。排尿日记、尿垫实验、咳嗽压力性试验等可对尿失禁的类型及程度进行诊断。盆底肌训练、电刺激治疗、药物治疗可在一定程度上缓解尿失禁,对于保守治疗失败的患者可考虑行尿道填充注射、男性吊带手术、外部压迫装置量植入等手术治疗方法,干细胞治疗SUI处于起步阶段,需进一步验证治疗效果。

Nowadays, Surgery remains the most effective way in treating benign prostate hyperplasia. However,any surgical method can cause urinary incontinence of different degrees and proportions. Postoperative scarring, combined with the compression of sphincter, destroyed the mechanism of urethral mucosa closure, causing postoperative stress urinary incontinence(SUI). Voiding diary, pad test and cough stress test are the main methods in diagnosing urinary incontinence's type and degree. Pelvic floor muscle training, electrical stimulation treatment, medication on the other hands can alleviate urinary incontinence. For patients in whom conservative treatment is ineffective, urinary incontinence may be treated by injection of urethral bulking agents, male sling surgery and implantation of external compression device. Stem cell therapy SUI is showing promises but needs further validation.
