目的

探讨应用Allium 金属覆膜输尿管支架治疗中长段输尿管狭窄的安全性和有效性。

方法

回顾性分析南京医科大学附属老年医院泌尿外科2020 年10 月至2021 年12 月间收治的23 例(30 侧)中长段输尿管狭窄患者的资料。

结果

经术中输尿管逆行造影和输尿管镜探查,明确狭窄段长度为3~15 cm,其中单侧狭窄16 例(16 侧),双侧狭窄7 例(14 侧)。采用输尿管镜加X线引导结合的技术方式,30 侧均扩张狭窄完成输尿管成形并同期植入Allium 金属覆膜支架。术后随访34~48 个月,20 例(86.96%)共27 侧输尿管支架植入后均效果良好。

结论

采用输尿管镜和X 线引导结合的技术方式,提高了输尿管狭窄腔内成形的成功率,Allium 金属覆膜支架的植入一定程度上解决了中长段输尿管狭窄扩张成形后持续定形的技术难题。

Objective

To discuss the safety and efficacy of Allium ureteral stent in the treatment of middle-long segment ureteral stricture.

Methods

The data of 23 patients with ureteral stricture of middle-to-long segments who were admitted to the Department of Urology of Geriatric Hospital of Nanjing Medical University from October 2020 to December 2021 were retrospectively analyzed.

Results

The ureteroscopy and retrograde ureteroscopy revealed that the lengths of the segments were 3-15 cm respectively,including 16 cases of unilateral stricture and 7 cases of bilateral stricture.Four steps of retrograde ureterography,ureteroscopy,dilatation of ureteral stricture and ureteral stent implantation were successfully completed under the guidance of C-arm X-ray.All of the 23 cases of dilatation of ureteral stricture were completed with the implantation of Allium ureteral stent.20 cases (86.96%) had good results after ureteral stents were followed up for 34-48 months.

Conclusion

The success rate of endoplasty of ureteral stricture can be improved by the combination of ureteroscopy and X-ray guidance,and the implantation of Allium ureteral stent solves the technical problem of continuous shaping of middle and long segment ureteral stricture after dilatation to some extent.
