目的

探讨微波热疗联合三联药物膀胱灌注治疗膀胱疼痛综合征/间质性膀胱炎(BPS/IC)的临床效果与安全性。

方法

回顾性分析2014年2月至2017年9月收治的51例BPS/IC患者的临床资料,其中女44例,男7例,平均年龄36岁,平均病程32个月。对照组采用单纯三联药物膀胱灌注,治疗组在膀胱灌注的基础上运用微波热疗治疗。所有患者治疗前后均行O'Leary-Sant间质性膀胱炎症状指数和问题指数评分(ICSI/ICPI),盆腔疼痛及尿频评分(PUF)和VAS疼痛评分以评估疗效。

结果

经治疗后,治疗组患者ICSI、ICPI、PUF及VAS评分均优于对照组[(7.3±3.3) vs(10.7±2.5)、(5.2±3.5)vs(7.5±3.5)、(19.6±3.4)vs(22.3±4.7)、(5.0±0.7)vs(5.8±1.1),P值均<0.05)],且两组患者治疗后各项评分均优于治疗前(P<0.05),治疗组患者有效率为91.7%,所有患者均未发生严重不良反应。

结论

微波热疗联合药物膀胱灌注治疗BPS/IC的疗效肯定,适合广泛开展,为临床治疗BPS/IC提供了一种有效方法。

Objective

To evaluate the efficacy of microwave thermotherapy combined with tri-drug bladder perfusion for treatment of bladder pain syndrome/interstitial cystitis (BPS/IC).

Methods

During Feb 2014 to Sep 2017, 51 BPS/IC patients were treated, including 44 women and 7 men, with mean age of 36 years, mean course of 32 months. The control group was treated with triplex drug bladder perfusion, and the study group was treated with microwave thermotherapy on the basis of bladder perfusion. O'Leary-Sant questionnaire interstitial bladder inflammation shape index and problem index score (ICSI+ICPI), pelvic pain and urinary frequency score (PUF), and visual analogue scale (VAS) were observed and recorded for IC/BPS symptom assessment before and after treatments.

Results

After the treatments, the rate of curative effect was significantly higher than that of the control group, and the difference was significant (P<0.05). ICSI, ICPI, PUF and VAS scores of study groups were improved than those of control group [(7.3±3.3) vs (10.7±2.5), (5.2±3.5) vs (7.5±3.5), (19.6±3.4) vs (22.3±4.7), (5.0±0.7)vs (5.8±1.1), all P<0.05)]. And all scores of the two groups after treatmant were improved than those before treatment. The effective rate of the treatment group was 91%, and no serious adverse reactions occurred in all patients.

Conclusions

Treating BPS/IC patients with microwave thermotherapy combined with intravesical drug therapy can effectively relieve clinical symptom.
