目的

初步评价覆膜网状支架(Allium)置入治疗输尿管梗阻的疗效。

方法

回顾性分析2018年10月至2019年5月中山大学附属第六医院泌尿外科收治的11例因良恶性病因导致输尿管梗阻患者资料。

结果

11例患者均行逆行Allium支架置入术,7例为单侧梗阻,并于患侧置入Allium支架1条,置入成功率100%;4例双侧梗阻中,3例双侧置入Allium支架各1条,置入成功率100%,1例双侧置入Allium支架各2条,置入成功率100%。1例因术后15 d复查CT及顺行造影提示患侧积水较前进展,梗阻仍然存在,二次手术取出患侧输尿管Alllium支架1条并重新置入Allium支架1条。所有患者治疗后临床症状明显改善,血肌酐、血尿素氮、血尿酸及肾盂分离值四种指标均较治疗前有显著性改善。所有患者均无支架相关疼痛及尿路症状。

结论

覆膜网状支架(Allium)治疗因恶性肿瘤引起的输尿管梗阻及多次频繁更换输尿管支架的非恶性输尿管梗阻临床疗效满意,应根据个体病情酌情选择。

Objective

To evaluate the efficacy and complications of (Allium) stent implantation in the treatment of benign and malignant ureter obstruction.

Methods

The retrospective study enrolled11 patients (including 5 males and 6 females) with ureter obstruction caused by benign and malignant causes treated in our hospital from October 2018 to May 2019.

Results

Allium stent implantation was performed in11 patients successfully. 1 Allium stent was placed on the affected side with a success rate of 100% in 7 patients. In 4 patients with bilateral obstruction, Allium stent was placed in 3 patients with 1 Allium stent per side and the implantation success rate was 100%; and 2 Allium stent per side in one patient and the implantation success rate was 100%. In one case, CT and anterogram reexamination 15 days after the operation indicated that the hydrocephalus on the affected side was advanced and the obstruction was still present. After the second operation, 1 ureteral Alllium stent was removed from the affected side and new Allium stent was re-implanted. The clinical symptoms of all patients were significantly improved after treatment, and the indexes of serum creatinine, blood urea nitrogen, blood uric acid and renal pelvis separation value were significantly improved compared with that before treatment. None of the patients had stent-related pain or urinary tract symptoms.

Conclusion

The clinical effect of covered reticular stent (Allium) in the treatment of ureter obstruction caused by malignant and non-malignant factors is satisfactory, and it should be selected according to the individual condition.
