目的

探讨前列腺增生(BPH)合并HIV感染/AIDS患者的前列腺组织炎症状态,并评估抗病毒治疗对其的影响。

方法

回顾性分析本院2018年6月至2024年6月收治的BPH合并HIV感染/AIDS手术患者的临床资料,根据患者术前是否抗病毒治疗将患者分为两组,分别为未治疗组及治疗组,未治疗组HIV-RNA水平明显升高,治疗组HIV-RNA水平低于检测下限,每组各12例,比较两组患者前列腺组织炎症程度及免疫细胞浸润情况。

结果

两组患者前列腺组织均可见不同程度的炎症细胞浸润,未治疗组前列腺组织中、重度炎症患者10例(83.33%),治疗组前列腺组织中、重度炎症3例(25.00%),两组差异具有统计学意义(P<0.05)。在炎症反应明显的部位,无论在腺体、腺周或基质,可见大量的CD4+T细胞、CD8+T细胞浸润,并以CD8+T细胞浸润为主。

结论

BPH合并HIV感染/AIDS患者往往合并前列腺炎,抗病毒治疗能一定程度降低前列腺组织的炎症水平。

Objective

To investigate the inflammatory status of prostate tissue in patients with benign prostatic hyperplasia (BPH) complicated by HIV infection/AIDS and to evaluate the impact of antiviral therapy on prostatitis.

Methods

A retrospective analysis was conducted on the clinical data of surgical patients with BPH and HIV infection/AIDS treated in our hospital. Based on whether the patients received preoperative antiviral therapy, they were divided into two groups: the untreated group and the treated group. The untreated group had significantly elevated HIV-RNA levels, while the treated group had HIV-RNA levels below the detection limit. Each group consisted of 12 patients. The degree of prostate tissue inflammation and immune cell infiltration were compared between the two groups.

Results

Both groups exhibited varying degrees of inflammatory cell infiltration in the prostate tissue. The untreated group had a moderate to severe inflammation rate of 83.33%, compared to 25.00% in the treated group, indicating a statistically significant difference (P<0.05). In areas with significant inflammatory responses, whether in the glands, periglandular regions, or stroma, a large number of CD4+ T and CD8+T cells were observed, with CD8+T cells being the predominant infiltrating cells.

Conclusion

Patients with BPH and HIV/AIDS often have concurrent prostatitis, and antiviral therapy can to some extent reduce the inflammatory levels in prostate tissue.
