Correlation between bladder wall thickness, detrusor wall thickness and overactive bladder in elderly women

Abstract:

Objective

To explore the correlation between bladder wall thickness (BWT) and detrusor wall thickness (DWT) measured by transvaginal ultrasound and overactive bladder (OAB) in elderly women.

Methods

A total of 80 elderly female patients with frequent urination and urinary urgency treated in Urology outpatient of Chongming Branch of Xinhua Hospital Affiliated to Shanghai Jiaotong University were enrolled as observation group from December 2021 to May 2022. All patients underwent pressure-flow study (PFS). The conditions of patients were graded according to OAB symptom score (OABSS). The presence or absence of detrusor overactivity (DO) and urinary incontinence (UI) in PFS process was observed. A total of 80 healthy elderly women undergoing physical examination during the same period were enrolled as control group. The relationship between BWT, DWT and OAB was analyzed.

Results

BWT and DWT in observation group were significantly thicker than those in control group (P<0.001). With the aggravation of disease severity, BWT and DWT were increased (P<0.001). BWT and DWT were positively correlated with OABSS (r=0.627, 0.556, P<0.005). PFS showed that among the 80 patients OAB, there were 23 cases with UI and 57 cases without, 31 cases with DO and 49 cases without. The BWT and DWT values of patients with UI were higher than those of patients without (P<0.05). BWT and DWT in OAB-DO patients were thicker than those without DO (P<0.05). BWT and DWT were of diagnosing value for OAB-DO (P<0.05), the AUC values were 0.668 and 0.713, respectively. The cut-off value, sensitivity and specificity of BWT and DWT were (5.91 mm, 41.94%, 89.80%) and (2.52mm, 51.61%, 97.96%), respectively. There were 21 cases with UI and 10 cases without in 31 patients of OAB-DO. The BWT and DWT values of patients of OAB-DO with UI were higher than those of patients without UI (P<0.05).

Conclusions

Transvaginal ultrasonography is a reliable diagnostic method, with low invasion. It can directly measure BWT and DWT, which has certain clinical evaluation value for the severity of OAB in elderly women.
